REQUEST FOR PSYCHOLOGICAL TESTING

Thank you for your intrest in our psychologial testing. We are currently accepting the following insurance: Regence BCBS, Moda, Providence, and PacificSource. Self-pay is an option for those without OHP and we can bill out-of-network for UnitedHealthcare (UHC) and Aetna.  Unfortunately we are unable to accept OHP primary or secondary for psychological testing at this time.
